Title: Relentless Readiness – A model for safety, but not for the fainthearted

The “New View” of safety has introduced transformative approaches to risk management, emphasizing adaptability and resilience. However, recent trends show the resurgence of outdated practices, such as the Energy Wheel and rebranded variations of Cardinal/Golden Rules, such as Sticky’s and HECA’s. These approaches risk undermining progress by perpetuating a narrow focus on engineering solutions while neglecting the human element and the aspirational drive that has historically propelled human achievement.

This presentation critiques the passive acceptance of mediocrity under the guise of inevitability, challenging the notion that human error must lead to systemic failure. Instead, it advocates for a new definition of safety: Relentless Readiness—a proactive state where culture, practices, and systems are continuously prepared to respond dynamically to emerging risks. It is the flip side of Chronic Unease.

‘Relentless Readiness for Risk (or R3) – as a definition, model and system, integrates the structured discipline of Safety I with the adaptive flexibility of Safety II, creating a hybrid approach that bridges the gap between prevention and resilience. It equips organizations with real-time risk management capabilities while fostering a culture of aspiration and peak performance. R3 redefines safety as an active pursuit of excellence, offering a robust framework for organizations aiming to thrive in uncertain and complex environments.

Abstract
The goal of safety is eliminating workplace fatalities. To achieve this, we must prevent and predict risks. Yet, despite extensive efforts, success remains elusive. While we can identify the risks and understand how they occur—such as high-energy work—we cannot reliably predict when or where they will happen. This unpredictability highlights two challenges:

  1. Traditional systems for identifying serious injury and fatality (SIF) risks rely on hindsight, analyzing past accidents or near misses.
  2. Static identification tools like audits, inspections, and risk registers fail to capture dynamic, evolving risks as they emerge.

This presentation introduces a new approach: “randomized readiness for risk” response, redefining safety the safety approach. Rooted in a 3-year Elimination of Fatalities program within a global corporation, this model demonstrates how operational processes can eliminate latent accident triggers and move safety into a predictive and preventative paradigm.

Abstract
Culture is a powerful force that shapes and influences behaviors at every level of an organization. However, when it comes to safety, the term “safety culture” may not fully capture the essence of what truly drives safety behaviors within a workplace. A more accurate term might be “risk culture,” as it better reflects how employees perceive, manage, and make decisions about risks in their daily operations.

This presentation will offer an in-depth exploration of what is commonly referred to as safety culture, focusing on how it is systematically created, the ways in which it impacts safety performance, and the different ‘types’ of culture that exist within organizations. A critical aspect of the presentation will be the measurement of these cultural elements, providing attendees with insights into how to assess and enhance their organization’s risk culture.

To build a powerful and effective risk culture, the presentation will outline six key pathways:

  1. Establish values: Establishing a common understanding of safety goals and risk management priorities that align with the organization’s overall mission.
  2. Set share vision: Motivating and engaging employees at all levels to take ownership of safety and risk management practices.
  3. Dare to differ: Encouraging innovation and creative problem-solving, even when it challenges the status quo.
  4. Empower the Frontline: Giving frontline workers the authority and tools they need to manage risks effectively and make informed safety decisions.
  5. Touch Hearts: Creating an emotional connection to safety that transcends mere compliance and fosters a genuine commitment to protecting oneself and others.
  6. Create readiness: Building a culture of care where the well-being of employees is prioritized, and safety is seen as a fundamental value.

In addition to these pathways, the presentation will also address the four ‘states’ of paralysis that can hinder organizational effectiveness:

  • The Paralysis of Fear: Where fear of making mistakes or facing repercussions stifles innovation and open communication.
  • The Paralysis of Protection: Where excessive caution leads to over-protection and limits proactive risk management.
  • The Paralysis of Pretention: Where organizations put up a facade of safety without truly integrating it into their culture and daily practices
  • The Paralysis of Atrophy: Where complacency and a lack of continuous improvement cause safety practices to stagnate over time.

Many organizations are unaware of the cultural state in which they operate and may not fully understand the “unwritten rules” that govern their approach to safety and risk management. This presentation will help attendees identify which state of paralysis their organization might be in and provide practical strategies for overcoming these barriers.

By attending this session, safety professionals, leaders, and managers will gain a deeper understanding of the dynamics that drive their organization’s risk culture. They will learn how to implement the six pathways to create a powerful, proactive culture that not only meets safety standards but also enhances overall organizational performance. The presentation will equip participants with the knowledge and tools needed to transform their organization’s approach to safety, making it a fundamental part of their operational DNA. As part of a multi-year intervention at a large mining corporation, Corrie Pitzer developed and deployed a dynamic ‘high reliability system’ with surprising results and success. He will discuss lessons on high reliability theory from the field and the “Deep Safe Organization”.

Following ‘deepdive risk discussions’ with 8000 front line workers in dangerous jobs, discovering over 2000 fatal risks exposure, and after reviewing over 10000 reports of potential Serious Injury, Fatal or Catastrophic (SIFC) risks, a new ‘model’ for risk emerged: The Random, Chaotic, Complex risks that lurks in the depth of all organizations – waiting to be discovered, or to explode in events.


Corrie gained new insights from what he calls the” 3rd Layer of Risk”, where not only the Black Swans live, but also the Purple Dragons, the Yellow Elephants and the mysterious White Slugs.
